Counselor/Social Worker
Job Duties and Responsibilities:
- Identify students with excessive absences; develop and implement plans that address barriers to daily attendance
- Collaborate with other districts to assist new students and families with obtaining required documents and completing registrations
- Identify and locate Low-Show and No-Show students for the September 30th Count and collaborate with school office staff to document information for the Unit Count
- Coordinate with building secretaries to ensure the accuracy of student attendance
- Identify obstacles that are impacting student attendance and learning (food, housing, technology, etc.) and assist in removing these barriers
- Refer students and families to school and community-based programs (CFF Community Schools, Dual Generation Center, and Center for Child Development, etc.) to provide necessary supports and services

The Center for Childhood Communication (CCC) of the Children’s Hospital of Philadelphia is seeking a licensed provider for health and behavior services and a bachelor’s level Social Work Care Coordinator for children, adolescents, and caregivers who are Deaf and/or Hard of Hearing (for Social Work position also with children with only speech language disorders). These staff will work as part of a team of audiologists, speech-language pathologists, educators, and other behavioral health providers. In addition to managing patients under the umbrella of the CCC, staff provides services in multi-disciplinary clinics. As a Behavioral Health Specialist you will be responsible for the development, implementation, monitoring and revision to the Person Centered Plan in conjunction with the interdisciplinary healthcare team.

- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers;
OR
- A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

PAHrtners Deaf Services is now accepting applications for the Direct Support Professional position at locations in and around Moon and Glenside. This role was formerly called Residential Support Coach (RSC). Various shifts available (full-time, part-time or flex).
Description:
This position provides direct services to adults with intellectual disabilities and behavioral health needs living in our residences. Responsibilities include: assisting with daily living skills instruction and training; aiding in community integration; transportation; medication administration and observation; writing progress notes. All shifts are available (day, evening, overnight and weekend).
Qualifications:
HS diploma; valid driver’s license; fluency in ASL or strong willingness to learn; demonstrated competency in independent living skills; ability to teach, advise and motivate Deaf consumers; strong interpersonal skills. Certified as a Peer Specialist, a plus!
